This form is intended for F-1 students requesting a SEVIS completion or termination.
- F-1 students seeking to withdraw should use the Withdrawal Request.
- F-1 students who have been approved a change/adjustment of status should use the Immigration Status Update Portal.
- F-1 students seeking to enroll in a different institution should complete the SEVIS Record Transfer-Out Form.

What is the difference between a SEVIS completion or termination?
The SEVIS completion process is used to indicate that a student has graduated and completed their course of study or completed OPT/STEM OPT. This will change the student's SEVIS status to "Completed." This process should only occur in cases where a student has completed their program and has departed the US.
The SEVIS termination process is conducted by a DSO who is responsible for terminating a student's SEVIS record upon learning that the student is no longer maintaining their F-1 status.
Student may be terminated when failing to maintain their F-1 status (such as, but not limited to, unauthorized drop below a full-time course load or working off-campus without proper authorization) or when students decide to forfeit their status (departing the US prior to the completion of the academic program).
Students who have violated their F-1 status may be eligible to apply for reinstatement. See Reinstatement to Active F-1 Status.

Are there any ramifications to my F-1 record if I choose to discontinue my study after this semester?
No. Students who plan to depart the US at the end of the semester, prior to the completing their academic program can do so without jeopardizing their SEVIS record.
Students who plan to depart the US in the middle of the semester must complete a withdrawal request from Texas State. This action will result in the termination of the SEVIS record.

I did complete the semester, but now I’ve decided I want to take some time off. When do I need to depart the US?
F-1 regulations require continuous full-time enrollment every fall and spring semester. Choosing to not enroll in a fall or spring may result in the termination of your SEVIS record. Please schedule an appointment with a DSO.

I've decided to go home before the semester starts. How do I withdraw from all my classes?

If my SEVIS record is terminated do I need a new I-20?
Yes, if your SEVIS record is terminated as a result of failure to maintain your F-1 status you must request a new I-20.
You may be eligible to apply for reinstatement of your F-1 status. Review the reinstatement website and schedule an appointment with a DSO.

I could not find a job and have reached my limit of unemployment days, what should I do?
You must depart the US as you are out of status. Remaining in the US while out of status may result in accrual of unlawful presence. Complete the SEVIS Completion or Termination form and include your departure or flight itinerary.
